MySparrow Terms and Conditions

Acceptance of these MySparrow Terms and Conditions is required to access your medical information or the medical information of another patient as a proxy through MySparrow.

What is MySparrow?

MySparrow is an Internet application that provides Sparrow patients and designated proxies with the following on-line services:

  • Secure and convenient way to access portions of your Sparrow electronic medical record. MySparrow does not contain your complete medical record. It contains only certain, key information about patients, such as health conditions, appointment reminders, medications, allergies, and test results.
  • A secure and convenient way to send non-urgent health-related messages to the patient's Sparrow physician. All messages sent through MySparrow will be sent to the electronic medical record for processing.
  • A secure and convenient way to receive test results and other health-related information from Sparrow.
  • A secure and convenient way for patients to schedule Sparrow appointments and view their upcoming Sparrow appointments.

Sparrow may add other features to MySparrow not described above. You will be notified about new features when they are made available to you.

Eligibility for MySparrow Access

MySparrow is available to Sparrow patients and designated proxies as follows:

  1. Adult patients 18 years and older.
    • Adults 18 years or older may submit an online request for to MySparrow, or point of service.
    • Adults 18 years and older or emancipated minors - proxy access to the MySparrow account of another adult or emancipated minor with permission* of the MySparrow account holder.
    • Adults 18 years and older or emancipated minors - proxy access to a MySparrow account regarding an incapacitated adult or emancipated minor if the court ordered legal guardian of the incapacitated Sparrow patient.
  2. Adolescents ages 12-17 years.
    • According to State and Federal law, adolescent patients between 12-17 years can access certain types of care without their parents' knowledge or permission. Sparrow shall provide adolescent patients with a MySparrow account if the patient, parent (or legal guardian), and provider can come to an agreement regarding parental proxy. Sparrow must ensure adolescents consent to parents or legal guardians gaining access to their MySparrow account, which could potentially contain information related to that care.
    • Parents, legal guardians or other legally authorized representatives of adolescents' ages 12-17 years with written permission of the adolescent may be granted proxy access*, unlessthe adolescent is incapacitated, developmentally disabled, or is not permitted to make healthcare decisions by court order.
    • Healthcare providers are not required to meet with adolescent patients without their parents, except for those situations protected by law.
  3. Minors ages 0-11 years
    • MySparrow is available to the parents and legal guardians of minor patients 0-11 years of age
    • Proxy access is terminated on the child'12th birthday.

* Permission granted by completing the Proxy Access portion of the MySparrow Consent Form and submitting it to Sparrow.

Patient Responsibilities for Use of MySparrow

You agree to observe the following requirements when using MySparrow:

  • Patients will not share their MySparrow username and password with anyone. If they do, that person will be able to access their medical record, and act on the patient's behalf, through MySparrow. Patients are solely responsible for any sharing of MySparrow content that they intentionally or unintentionally communicate to others. The patient shall agree to change their MySparrow password immediately if they believe their password has been compromised in any way.
  • Emergency questions should not be submitted through MySparrow. Call your physician office, 911 or go to the nearest Emergency Room. Typical turnaround time for responses is three (3) business days.
  • Patient should be aware that messages through MySparrow may be accessed by clinical support staff before going directly to the physician. This may include any sensitive information that you send.
  • Messages through MySparrow that contain profanity, threats or libelous references may be used as grounds for automation termination of MySparrow access.
  • MySparrow users will be notified via E-mail when there is new medical information to be viewed on MySparrow.

Proxy Activation Procedures

Patients or adults may be granted proxy access will receive instructions and a PIN number via their preferred method of communication. (e.g. letter, telephone or email. Sparrow PIN numbers will be disabled after 60 days).

Having a MySparrow proxy does not mean that the proxy can look at all of Sparrow medical records. The proxy can only look at the information in MySparrow. The patient cannot limit the information in MySparrow that the proxy can look at.

Adult Proxy Access

Proxy access to the MySparrow record of an adult will be granted once the patient signs and completes the Proxy Access Adult form and an authorization for release of medical information using Sparrow for HP-02 or, if the patient is incapacitated, the person requesting to be proxy must be the court ordered legal guardian of the incapacitated adult.

The patient's MySparrow record will be accessed through the proxy's MySparrow record. Completion of the Proxy form will establish a MySparrow record for the proxy and for the patient. The patient and the requestor must both complete the electronic agreement for establishing the Adult Proxy process.

Adolescent Proxy Access

An Adolescent that has been given access to MySparrow is not required to have a proxy.

If the parent or legal guardian does not have a MySparrow account, one will be activated in order to grant proxy access.

Pediatric Proxy Access

Pediatric proxy access for MySparrow allows a parent or guardian to securely communicate on behalf of a Sparrow pediatric patient who is under the age of 12 and access components of their medical record. Access can be requested at the child's next appointment, at the point of service or by the Health Information Management Department.

  • The proxy requestor must be the parent or legal guardian of the pediatric patient.
  • If the proxy requestor doesn't have a MySparrow account, one will be activated in order to grant proxy access.

Discontinuing Use of MySparrow

You can discontinue use of MySparrow at anytime by contacting your physician's office and asking them to deactivate your account.

If we determine in our sole discretion that you have violated these MySparrow Terms and Conditions and/or abused the use of MySparrow, either as a patient or as a proxy, we reserve the right to discontinue your use of MySparrow at any time.

Proxy access can be terminated, online or by written request, if access disputes between parents or between parent and patient cannot be resolved.

Web Site Terms and Conditions and Security Policy Incorporated

These MySparrow Terms and Conditions are governed by and construed in accordance with the laws of the State of Michigan. The Sparrow Health System Website Terms and Conditions of Use as set forth at and the Sparrow Health System Website Security Policy as set forth at are incorporated into these MySparrow Terms and Conditions as if set forth herein. We may modify these agreements at any time, and you agree that such modifications are effective immediately upon posting of the modified version.
